Consultation response: Framework for Learning and Improvement from Patient Safety Incidents

NICCY welcomes that the Department has undertaken a consultation on the introduction of a new Regional Framework for Learning and Improvement from Patient Safety Incidents (PSI) to replace the current Serious Adverse Incident Procedure in NI.

Commissioner Chris Quinn provided a response to the consultation about the Framework for Learning and Improvement from Patient Safety Incidents.

It is NICCY’s position that this framework is the foundational pillar to assess and evaluate healthcare services within the child’s rights context, and further, extends to the assessment and evaluation of child mortality during the provision of such services.

We are disappointed that no Child Rights Impact Assessment (CRIA) was undertaken which could have further examined the possible impact of the proposals on children and young people’s rights in NI an encourage the Department of Health (the Department) to conduct one. We also encourage the Department to engage directly with children and young people as part of this consultation process to adequately hear their views and experiences.

NICCY welcomes that the Department is undertaking this consultation on the introduction of a new Regional Framework for Learning and Improvement from Patient Safety Incidents (PSI) to replace the current Serious Adverse Incident Procedure in NI. Given our capacity however, we are not in a position to respond fully to the consultation but wish to highlight a particular area of concern for the office relating to Child Death Reviews given the Inquiry into Hyponatraemia-related Deaths Report Summary mentioned within the document.
